​Mandated Exchange Notice
Any employer subject to FSLA (Fair Labor Standards Act) per the Department of Labor must deliver the appropriate version of Form OMB No. 1210-0149 to new employees within 14-days of hire.

• Employers who offer a health plan to some or all employees:
